On 16 July 2026 we hosted our first Frailty Conference that brought together clinical and operational leaders from across our system to:
- Showcase the outstanding frailty work already happening in single and multiple neighbourhoods and places
- Share evidence of its impact — including innovative alternatives to Emergency Department attendance for frail people
- Identify our collective strengths and any gaps we need to address
- Build the shared understanding needed to scale and spread best practice, improving outcomes for our residents

Workshop Session One
East Cambridgeshire Integrated Neighbourhood and Ely Primary Care Networks (PCNs)
Hear how the Integrated Neighbourhood Frailty Programme is working in East Cambridgeshire, including the successes so far and Vision for what’s next from a at Practice, PCN and Neighbourhood level.

Find out more about the Enhanced Reviews in Care Homes (EnRICH) programme, and how it is bringing enhanced speciality care to care home residents using a proactive and Comprehensive Geriatric Assessment (CGA) led approach, that avoids hospital admissions by bringing specialist input to the patient rather than vice versa.
Health and Independent Living Support (HILS)
Hear from HILS, a charitable social enterprise who is now a significant provider of community-based support for people in Hertfordshire (and beyond). Join them to gain a non-statutory perspective on frailty services.

Immerse yourself in a session jointly run by Carers in Bedfordshire and Carers in Hertfordshire which is focused on carers and their experiences of supporting people with frailty in this voluntary sector led session.

Workshop Session Two
Hospice perspective
Join colleagues from Isabel Hospice and Keech Hospice who will share insights and case studies about how they’re supporting patients with frailty, end of life care and advanced illness. This session includes a case study about a patient who was successfully cared for in the community, the health economy impact and neighbourhood teamworking that was achieved for his care.

Learn more about how Community Frailty Group Clinics at Manor View have supported their local residents, and how they are now a Regional Vanguard for the East of England region.

Bedfordshire Hospitals NHS Foundation Trust (BHFT)
An acute Trust perspective on the delivery of a frailty service that the team at Bedfordshire Hospitals NHS Foundation Trust (NHFT) have developed and delivered in their local area, and the impact it has had for local people.
Chiltern Hills PCN and The Bridge PCN
Listen to two different PCN approaches to frailty including how they have rolled out frailty related pathways and taken a proactive, multidisciplinary care approach, alongside the use of technology to improve mobility and reduce falls risk.

Hertfordshire Community NHS Trust (HCT)
A Community Trust perspective on frailty, including examples of best practice (supported by data) that they have implemented.

Central London Community Healthcare Trust and West Hertfordshire Teaching Hospital
A workshop of three parts – focusing on their frailty virtual hospital; care coordination at West Hertfordshire Teaching Hospitals (WHTH) front door, and a social investment fund which supports a Macmillan funded proactive care model and an Alzheimer’s UK funded programme to support dementia in care homes to prevent admissions.

Present on the major focus on frailty work in Hatters Heath PCN for six years including being involved in setting up the at home first frailty service with East of England Community Health and Care NHS Trust (formerly CCS).

North West Anglia NHS Foundation Trust (NWAFT) – North Place Team
Find out how the North Place Team, hosted by North West Anglia NHS Foundation Trust, have focused on frailty multidisciplinary team working including Population Health Management (PHM), personalised care planning, MDT working and its positive impact. They will also cover structured medication reviews with this cohort, and virtual ward and urgent care response for urgent frailty admission avoidance.
